Memberships & Programs

This is all so different. Help me understand the different ways to work with you. 

We do function differently and we know it can take a minute to think outside the box when it comes to healthcare! Basically we have gracefully exited the insurance-care system in order to spend more time with you and create innovative and holistic offerings to support your Whole Health. This would be impossible via insurance. 

To help you budget out your care in a cost-effective way (often cheaper than if you used insurance!) and to eliminate barriers that cause gaps in care, a membership is the foundation of what we do. Once we are a line item in your budget, there’s no reason not to call us! We love to partner with patients who value a longitudinal relationship with their clinician, so it’s a perfect fit. 

If the commitment of the membership isn’t right for you right now, we developed two other options. Anyone can meet with us for a one time comprehensive consultation that you can take back to your care team for implementation. We will help you gather all the data needed beforehand, spend an hour with you and give you a Whole Health Plan. Naturally no prescriptions are written in this process since no follow up is planned, though you always have the option to convert to a membership if you choose.  

For those interested in focusing on optimized hormones or fat loss, there is also a ninety day program (this is not available for primary care since that naturally requires a longitudinal relationship).

  • Forever Fat Loss centers on metabolic health and sustainable fat loss (nutrition strategy, activity/fitness, behavior, meds when appropriate, group support) and is available for men and women, teens and above.


    Optimized Hormones centers on (peri)menopause symptom relief and long-term health (hormone evaluation, MHT when appropriate, non-hormonal options, symptom tracking). This naturally will include fat loss support if that is also a goal for you. 

    Both are available via one-time consults, 90-day programs and memberships.

    • Start unsure / want expert eyes on labs & symptoms? Book a one-time comprehensive consult → get a written plan and next-step options.

    • Want to get that comprehensive plan, but also have time and support to implement a plan and see progress on a clear goal (hot flashes, sleep, or jump-start fat loss)? Choose the 90-Day Program.

    • Want ongoing access, accountability, care continuity and the most cost-efficient way to partner? Choose a membership (Primary Care, Optimized Hormones, or Forever Fat Loss).

  • You’ll leave with a written self-care and professional care plan that includes lifestyle recommendations, supplementation, pharmaceuticals and further testing if applicable. You can: (1) implement it with your current PCP and healthcare team, (2) enroll in the 90-Day FIT Protocol, or (3) join an ongoing membership for continued support and access with no registration fee.

  • Three paths: (1) “graduate” with a maintenance plan and implement it with your current PCP and healthcare team, (2) roll into the matching membership for ongoing care/accountability with no registration fee, or (3) repeat another 90-day block for a new goal.

  • Yes—primary care services require a Primary Care membership. You can do Optimized Hormones or Forever Fat Loss while keeping your existing PCP.

  • Require registration fee: Primary Care Membership ($197), Optimized Hormones or Fat Loss Memberships ($197).

    No registration fee: 90-Day FIT Protocol ($597 paid at time of registration), One-time consult ($397 paid at time of registration).
    If you roll from one plan to another (example- one time consult to a membership) there is no registration fee.

  • The memberships (primary care, fat loss or hormone) all have a six month commitment.

  • Absolutely! These are specialty membership programs and you will want to keep your PCP for general/urgent care while we handle hormone, metabolic and longevity goals.

  • Absolutely! Your Whole Health Plan as a Primary Care Member can include fat loss & hormonal support.

Money, Insurance & Access

  • We don’t bill insurance for our professional services. Because our services don’t fit discreetly into the insurance box, we are unable to provide superbills/itemized receipts with the coding required for insurance reimbursement. . Many members use insurance for labs, imaging, and medications and we help you use your insurance benefits in the most cost-effective way. We never charge copays or deductibles and ironically we often decrease a patient's overall care cost.

  • Please refer to tax code experts for specific rules related to this, but we have many patients who successfully use their HSA or FSA accounts. We are registered as a qualified healthcare provider for these purposes and are able to provide documentation regarding medical necessity when needed.

  • Yes—group memberships with unlimited visits, easy access, and no copays for employees or group members are available. Please reach out for a personalized quote.

Care Experience

  • Direct contact for urgent issues after hours. We handle what we safely can to help you avoid unnecessary urgent care/ER visits.

  • Both—together we choose what’s medically appropriate: in-person, video, phone, or secure messaging.

  • One of the BEST things about GOZA is that it allows you to form a treatment plan with your clinician based on what’s medically appropriate (not what the insurance thinks is appropriate).  That means we can decide together if you need an in-person visit, a video chat or phone call, an email or even a home visit in some cases.  Of course, in order to diagnose and manage many conditions, nothing beats being face to face, but you and your doctor will decide that together.

Cost & Membership Perks

  • Yes! That is an added value to your membership. We offer standard in-office tests which are frequently costly with insurance (example - EKG, UA) at no extra charge, plus we help coordinate care outside the office for you (referrals, labs and imaging) using your insurance or at our negotiated discounted fees with outside providers. Example estimates when not using insurance: Standard annual lab testing $100, MRI of any body part $400, X-ray of any body part $85.

  • We help you coordinate medication costs in the cheapest way possible. Sometimes this means advocating for you with your insurance or other cost savings programs. Sometimes it means getting medications at wholesale rates and dispensing them in the office.

    For those interested in GLP-1 medications for fat loss support, our discounted cost ranges from $100-$400 per month, with the average being $200.

  • Yes! Your membership is worth much more than clinician visits. It has a whole gym membership + warehouse club membership effect. Members-only pricing on select supplements, medications, wellness devices (Emsculpt NEO, Emsella, Emface, Exion), workshops, group fitness (as available), retreats, and 1-1 wellness services are available for members.

  • With our direct access primary care membership, you pay $97/month for unlimited access, including in-office testing like strep, UA, EKG (free of charge), after-hours availability, and transparent pricing for labs/imaging ($100 for expanded annual wellness labs, $85 for any X-ray, $400 for any MRI). Because we avoid the usual insurance-driven overhead, referrals, and urgent care/ER default path, you often save significant dollars while getting more personalized service.

    Here’s how it stacks up in realistic scenarios — using a $3,000 deductible insurance plan typical in Central Ohio — so you can see how we could save you both money and hassle.

    Scenario 1: UTI

    Traditional Insurance Model: ~$325 out-of-pocket at urgent care for visit and UA meeting your deductible

    DPC Model: Included in your membership

    Priceless Aspects: You don’t have to suffer waiting to get in because you got a same day appointment or better yet we can do a quick telehealth visit!

    Scenario 2: Ankle injury + X-ray

    Traditional Insurance Model: ~$1,400 for an ER visit and an X-ray

    DPC Model: $85 imaging

    Priceless Aspects: We were able to get the xray and see you the same day. You’re evaluated by someone who actually knows!

I’m still not sure if GOZA Wellness is right for me. How do I know?

Your are mostly likely a GOZA patient if:

  • You believe that your relationship with your doctor should be a close one based on mutual trust and respect.

  • You are comforted by the thought that if something arises, your personal doctor will be available for an opinion, not only based on what’s going on at the time, but also knowing your medical history and life circumstances.

  • You are busy and want your health to fit into to your life in a more convenient way.

  • You are a think outside the box type of personality and are willing to adopt new processes that make sense to you even before large industries recognize their value.

  • You may not be a “techy”, but you are comfortable with the use of technology in other areas of your life and are willing to use newer technologies like email, messaging and video in addition to good old fashioned face to face visits and phone calls.

GOZA Wellness might not be the fit for you if:

  • You are content with seeing many different providers at urgent cares or minute clinics for small illnesses or injuries that come up.

  • You’d prefer to let large industries (like insurance or hospital systems) set the rules and wait on them to change them before you adopt new processes.

  • You trust that what your health insurance covers is the best healthcare you need.

  • You don’t have access to the internet or email and are not willing to explore this in addition to face to face visits.
